Child and Family Policy Center • $20,000
CFPC will continue its work in children's health policy development and implementation by way of organizational
support for expanded coalition building, public education and awareness, and grass roots as well as direct contact with
legislators and the Executive branches of state government. CFPC has been successful over the past 2 years in expanding
both content and quality of child health coverage in the state and is in a unique position to influence additional policy
development for health care reform while monitoring and playing a significant role in policy implementation.
Orchard Place/ Child Guidance Center

Early Childhood Mental Health Consultation is a problem-solving and capacity building intervention implemented within a
collaborative relationship between a professional consultant with mental health expertise and one or more professionals,
primarily child care or early education, who have other areas of expertise. ECMHC aims to improve ability of child care
program staff, K-3rd grade teachers, families, programs and systems to prevent, identify, treat and reduce the impact of
mental health problems among young children and their families. Project expands the scope of current program to include
transition from pre-school to early elementary grades, assist childcare and preschool programs to set up and implement
social and emotional developmental screening, and training for centers in Positive Behavior Supports.
Primary Health Care, Inc. •
$53,300
Diabetes Pro-Active Care Project is part of a larger project to move much of the primary care of chronic
diseases such as obesity (and related conditions such as diabetes) out of the clinical office and into the home and
community settings. The Pro-Active Care coordination team focuses on such self-management items as diet, exercise,
health literacy, and keeping well. The community-based Care Manager will serve as a link between the patient, primary
medical home, and an array of community and public health resources needed for a comprehensive approach to chronic
disease. The Care Manager will visit the patient in the home, arrange community services relevant to the disease,
communicate directly with the primary care medical team and do simple procedures in the home in advance or following
an office visit that has been carefully coordinated to provide comprehensive care. The larger project internet-based
patient portal access to ask questions, receive lab reports and make "virtual office visits";
and a "health card" which can be loaded with credit for certain health-related services for the uninsured.
Healthcare Coverage for Kids Coalition (HCKC) •
$30,000
HCKC enrolls eligible children in Hawk-i (Iowa state children's health insurance program) and Medicaid. Outreach
specialists provide each family with individualized assistance needed to accurately complete an application, address
barriers, and follow the application from submission to enrollment. Children enrolled in Hawk-i experienced increased
access to routine preventive care, needed specialists, preventative health guidance and dental care, improved overall
health rating, and increased access to needed vision, behavioral and emotional care.
Iowa Health Foundation - La Clinica •
$30,000
Apoya su Salud Integral is a depression care program within the full primary care continuum provided by La Clinica de la
Esperanza. The care management program incorporates an evidence-based mental health intervention program designed
particularly for use in primary care settings which has demonstrated positive results in helping patients identify mental
health issues, namely depression and anxiety, and involving them in the treatment process.
Polk County Health Department - Healthy Polk 2010 •
$25,000
Every 10 years Healthy Polk, with assistance from the Polk County Health Department, develops a community health plan
for Polk County. Key components of the process included community conversations, identification of data sources,
development of proposed indicators, choosing priorities, and a community awareness campaign. Input was collected from
thousands of people who live and work in Polk County. Upon completion of the Healthy Polk 2020 plan, implementation
includes monitoring the community's progress and providing regular updates to the community, convening Town Hall meetings,
and serving as a catalyst for change by identifying areas that warrant greater attention. Healthy Polk will also
facilitate the organization of new work groups, task forces and campaigns to ensure all Polk County residents are
healthier in 2020 than they are today.
