Goal: Visiting Nurse Services will develop a partnership with providers, community-based organizations, Polk County Health Services and Iowa Medicaid Enterprise to address the needs of the community’s most vulnerable populations through the transition to Medicaid Modernization. VNS will bring together a strategic planning group to determine strategies for providers assisting consumers with navigating through the changes imposed by Medicaid Modernization. This project includes developing a planning group and disseminating critical information through outreach and education of Medicaid members.
